NUTRITION COUNSELING

For one-to-one appointments (Individuals):

1. Individualized Initial Nutrition Consultation (1 hour)
   Includes: an electronic diet nutrient analysis and a meal plan
2. Follow-up Visit (30 minutes)
   Reviewing nutrient analysis, providing additional education/ resources
3. Telephone or Skype Consult
4. Home Visit (1 hour)*
   Includes short travel (between the GTA, Markham, Newmarket, and Vaughan)*
   Includes: an electronic diet nutrient analysis and a meal plan
5. Interactive Grocery Tour (1 hour)
   Includes: a meal plan

NOW OFFERING NUTRITION PACKAGES

1. Nutrition Assessment Package:
   Includes:
   –  The 1-hour individualized initial nutrition consultation, and
   –  Two (2) 30-minute follow-up visits*

   Includes
   –  The 1-hour initial visit,
